The Silent Thief of Health and Longevity
We usually think of inflammation as something we can feel—a swollen ankle, an infection, a sore joint or a fever. In those situations, inflammation is actually a good thing. It is your body’s natural response to injury and an essential part of healing.
But there is another type of inflammation that concerns me much more when I think about long-term health:
Chronic, low-grade inflammation.
This type of inflammation can simmer quietly beneath the surface for years. You may not feel it, yet it can affect your blood vessels, brain, metabolism, joints, muscles and skin—and contribute to many of the chronic diseases we associate with aging.
Scientists sometimes use the term “inflammaging” to describe the relationship between chronic inflammation and aging.
In longevity medicine, one of our goals is to identify unnecessary inflammation early, determine what is driving it, and reduce it before years of damage accumulate.
How Inflammation Robs You of Your Health
Inflammation doesn’t affect just one part of the body. It changes the environment in which virtually every cell has to function.
Your Heart
Cardiovascular disease is not simply a cholesterol problem.
Inflammation plays an important role in atherosclerosis, endothelial dysfunction and the development and instability of arterial plaque. This is why I don’t evaluate cardiovascular risk with a standard cholesterol panel alone. I want to understand cholesterol, inflammation and metabolic health together.
Your Brain
Chronic inflammatory signaling can also affect the brain. Neuroinflammation is increasingly recognized as an important component of cognitive aging and neurodegenerative disease.
When we talk about preserving memory and reducing our future risk of cognitive decline, identifying and reducing sources of chronic inflammation should be part of that conversation.
Your Metabolism and Weight
One of the most important relationships is between inflammation and insulin resistance.
Visceral fat—the metabolically active fat surrounding our internal organs—can release inflammatory signals. Those signals can contribute to insulin resistance, which promotes additional fat storage and metabolic dysfunction.
The result can become a vicious cycle:
Inflammation → insulin resistance → visceral fat → more inflammation.
This is one reason losing excess visceral fat can have health benefits far beyond the number on the scale.
Your Muscles and Joints
Chronic inflammation can contribute to pain and stiffness and can work against healthy muscle function.
Maintaining muscle becomes increasingly important as we age because muscle is not simply about strength or appearance. It is one of our most important metabolic and longevity organs, helping regulate glucose, insulin sensitivity, mobility and resilience.
Your Skin
Inflammation can even accelerate what we see in the mirror. Chronic inflammatory and oxidative stress can contribute to collagen breakdown and declining skin quality, adding to wrinkles, laxity and loss of elasticity. Healthy aging really does occur from the inside out.
Where Is All This Inflammation Coming From?
This is where functional medicine becomes particularly useful.
Rather than simply identifying an elevated inflammatory marker, I want to know:
Why is it elevated?
Some of the most common contributors include:
- Insulin resistance and elevated blood sugar
- Excess visceral abdominal fat
- Highly processed foods and excess sugar
- Gut dysbiosis and gastrointestinal problems
- Chronic infections
- Periodontal disease
- Autoimmune conditions
- Smoking and excess alcohol
- Poor sleep and sleep apnea
- Chronic stress
- Hormonal and thyroid abnormalities
- Nutrient deficiencies
- Physical inactivity
- Environmental exposures
Often, there isn’t just one cause. Several smaller problems may be creating a cumulative inflammatory burden.
5 Labs I Use To Look For Hidden Inflammation
There is no single perfect test for inflammation. I prefer to look at patterns.
1. hs-CRP
High-sensitivity C-reactive protein (hs-CRP) is one of our most useful markers for detecting low levels of systemic inflammation and is also used in cardiovascular risk assessment.
2. Fasting Insulin
You can have a relatively normal fasting glucose while your pancreas is producing excessive insulin to keep it there. Elevated insulin can be an early clue that metabolic dysfunction is developing—sometimes years before diabetes appears.
3. Glucose + Hemoglobin A1c
These help us understand longer-term glucose regulation. Repeated glucose elevations and insulin resistance create metabolic stress that can promote an inflammatory environment.
4. Ferritin
Ferritin is commonly thought of as an iron-storage test, but it can also rise as part of an inflammatory response. That is why ferritin should never be interpreted by itself. I evaluate it along with iron studies and the rest of the clinical picture.
5. Advanced Cardiovascular Markers
Because inflammation and cardiovascular disease are so closely interconnected, I frequently look beyond conventional cholesterol testing.
Depending on the patient, this may include ApoB and lipoprotein(a), or Lp(a), along with other advanced cardiovascular testing.
These are not all inflammation markers themselves. Instead, they help us understand whether inflammation is occurring in the setting of other important cardiovascular risks.
How Do We Lower Inflammation?
This is one of the areas where functional medicine can be particularly powerful. The goal isn’t simply to take something that lowers an inflammatory marker.
The goal is to identify and remove the reasons your body is producing excessive inflammation in the first place.
Control Glucose and Insulin
Reducing excess sugar and refined carbohydrates, increasing protein and fiber, exercising and improving insulin sensitivity can dramatically change the metabolic environment. For some patients, simply discovering which foods are producing unexpected glucose spikes can be transformative.
Reduce Visceral Fat
Fat around the abdominal organs is metabolically active and can produce inflammatory signaling molecules. Reducing visceral fat is therefore not simply about looking thinner. It can be an important anti-inflammatory intervention.
Build and Maintain Muscle
I cannot emphasize this enough. Strength training is longevity medicine. Muscle improves insulin sensitivity, helps dispose of glucose, protects bone, supports metabolic health and allows us to remain strong and independent as we age.
Eat an Anti-Inflammatory Diet
Think less about a temporary “diet” and more about building an anti-inflammatory nutritional pattern. Emphasize colorful vegetables, berries, high-quality protein, omega-3-rich fish, extra-virgin olive oil, nuts, seeds, herbs, spices and fiber-rich whole foods. Reduce added sugars, refined carbohydrates and highly processed foods.
Restore Gut Health
If symptoms suggest a gastrointestinal problem, we may investigate digestion, the microbiome, intestinal infections or other potential sources of immune activation. The key is targeted testing—not ordering every available test.
Fix Your Sleep
Poor sleep is inflammatory. It also worsens insulin resistance, cortisol regulation, appetite and weight control. Seven or eight hours in bed does not necessarily mean seven or eight hours of restorative sleep. Snoring, sleep apnea, frequent waking and poor sleep quality deserve attention.
Address Chronic Stress
Stress has real biological consequences. Chronic activation of the stress response affects cortisol, glucose, blood pressure, sleep, appetite and immune signaling. Exercise, meditation, breathing practices, time outdoors, social connection and simply creating periods of genuine recovery can all matter.
Optimize Hormonal and Metabolic Health
Hormones, metabolism and inflammation are deeply interconnected. Depending upon the individual, evaluating thyroid function, menopausal hormones, insulin resistance and other metabolic factors may uncover important pieces of the inflammatory puzzle.
Use Supplements Strategically
Omega-3 fatty acids, magnesium, vitamin D, curcumin and certain antioxidants or phytonutrients may be useful for selected patients. But supplements are just that—supplemental. No capsule can compensate for uncontrolled blood sugar, visceral obesity, five hours of sleep, chronic stress and an ultra-processed diet.
Don’t Just Lower the Number. Find the Cause.
If your hs-CRP or another inflammatory marker is elevated, I don’t simply want to make the laboratory result look better. I want to understand why your immune system is sending that signal. Is it coming from insulin resistance? Visceral fat? Poor sleep? Your gut? Periodontal disease? Chronic stress? An autoimmune process? Hormonal changes? Or is it a combination?
That is one of the fundamental differences in a functional medicine approach.
We aren’t only asking:
“What disease do you have?”
We are also asking:
“What is happening in your biology today that could become disease tomorrow?”
Reducing chronic inflammation isn’t just about feeling better now. It’s about protecting your heart, brain, metabolism, muscles, joints and skin for the decades ahead. Because the real goal of longevity medicine isn’t simply adding years to your life.
It’s adding healthy, active, cognitively sharp years to your life—and making those years worth living.
