The Silent Disease You Can’t Feel – Why Metabolic Syndrome
and Diabesity Are Quietly Stealing Your Health

Most people assume they would know if their blood sugar were abnormal. They expect diabetes to announce itself with obvious warning signs—constant thirst, frequent urination, fatigue, or blurred vision. The reality is very different. Long before diabetes is diagnosed, a slow and silent process may already be underway. Blood sugar begins to fluctuate, insulin levels rise, inflammation increases, and excess fat starts accumulating around vital organs. Because you can’t feel these changes, they often go unnoticed for years, quietly damaging the body long before a diagnosis is ever made.
This is the world of metabolic syndrome and diabesity—the dangerous combination of insulin resistance, excess body fat, and impaired glucose regulation that has become one of the greatest health threats of our time.
The statistics are staggering. Nearly 40% of American adults have prediabetes, and the overwhelming majority don’t know it. Even more concerning, research suggests that nearly 90% of U.S. adults have at least one marker of poor metabolic health, whether it’s elevated insulin, increased abdominal fat, abnormal cholesterol, elevated blood pressure, or impaired blood sugar regulation. These conditions don’t simply increase the risk of developing Type 2 diabetes—they dramatically increase the likelihood of heart disease, stroke, dementia, fatty liver disease, infertility, kidney disease, and several forms of cancer.
One of the biggest misconceptions I encounter is the belief that weight gain is simply the result of eating too much or exercising too little. While calories certainly matter, they are only part of the story. Hormones—particularly insulin—play a profound role in determining whether your body stores fat or burns it for energy.
Insulin is often referred to as the body’s primary fat-storage hormone. Every time we eat, insulin rises to help move glucose from the bloodstream into our cells. That is a normal and healthy response. The problem occurs when insulin remains elevated throughout the day because of excess calories, highly processed carbohydrates, chronic stress, poor sleep, inactivity, or simply years of metabolic dysfunction. When insulin remains chronically elevated, the body receives a constant signal to store energy rather than release it. As a result, many people find themselves eating less, exercising more, and still struggling to lose weight. They’re not lacking discipline—they’re working against a metabolism that has become resistant to change.
This is one of the reasons I pay close attention to fasting insulin, not simply fasting glucose. Blood glucose often remains “normal” for years because the pancreas works overtime to produce more insulin and keep glucose under control. By the time fasting glucose or hemoglobin A1c finally become abnormal, insulin resistance has often been quietly progressing for a decade or longer. In my practice, I strive for optimal, not merely normal, metabolic health. While laboratory reference ranges often define normal hemoglobin A1c as up to 5.6% or 5.7%, I generally aim for an HbA1c below approximately 5.2% whenever appropriate and a fasting insulin below 5 μIU/mL as markers of excellent insulin sensitivity and the ability to lose weight. These are individualized goals, but they help us identify metabolic dysfunction long before diabetes develops.
Elevated blood sugar affects far more than the pancreas. One of the most important processes most people have never heard of is glycation. You can think of glycation as sugar acting like a sticky coating inside the body. Glucose attaches to proteins, forming compounds known as Advanced Glycation End Products (AGEs). Over time, these compounds stiffen collagen, injure blood vessels, damage nerves, impair kidney function, contribute to cataracts, accelerate skin aging, and increase the risk of cognitive decline. In many ways, glycation represents one of the fundamental mechanisms of aging itself. The healthier we keep our blood sugar over a lifetime, the less opportunity there is for this slow accumulation of damage.
One of the most exciting advances in preventive medicine has been the availability of Continuous Glucose Monitoring (CGM). While originally developed for people with diabetes, CGMs have become an invaluable tool for anyone who wants to understand how their body responds to everyday life. A CGM measures glucose around the clock, allowing you to watch your metabolism in real time. Many patients are surprised to discover that blood sugar doesn’t just respond to food. A poor night’s sleep, emotional stress, intense exercise, alcohol, travel, illness, or even skipping breakfast can produce significant changes in glucose. Seeing these responses firsthand transforms abstract nutrition advice into highly personalized information. Instead of wondering whether a certain meal, workout, or stressful day affected your metabolism, you can actually see the answer.
Perhaps the most empowering aspect of this approach is that metabolic health is not determined by genetics alone. The body is remarkably adaptable when given the right environment. Improving nutrition, building muscle through resistance training, prioritizing sleep, managing stress, increasing daily movement, and reducing excess body fat all improve insulin sensitivity. Small changes, repeated consistently over time, can dramatically reduce inflammation, improve energy, support healthy weight loss, and lower the risk of many of the chronic diseases we once considered inevitable.
